Referring to us
There are separate referral processes for our two pain services.
Chronic pain services
We provide a national service accepting referrals from throughout the UK. Our clinical services are available to patients with all types of chronic pain. Our assessment criteria are only that an individual's pain should be persistent, disabling and not adequately helped by other treatments which may be available locally.
If you have already been seen at a pain clinic or have attended an outpatient programme and feel that your problems remain unresolved your GP or consultant can refer you to our service. Before suitable referrals can attend a programme, they attend a multidisciplinary assessment of their needs. This provides recommendations for future treatments.
Referrals can be made by GP or consultant via Choose and Book, provided the patient has already been seen at a secondary care pain clinic or similar, or in writing to:
Adult programme: Dr Hannah Connell, service lead children and adults or Peter Brook, consultant in pain management.
Adolescent programme or children's CRPS service: Dr Hannah Connell, consultant paediatric clinical psychologist; or Dr Jacqui Clinch, consultant in paediatric rheumatology and adolescent chronic pain.
We are also able to accept private patient referrals. For further information or referral advice, please contact Lisa Self, service manager, tel. 01225 473427; email firstname.lastname@example.org; fax 01225473461.
This is a national service accepting referrals from across the UK. Referrals can be made via choose and book or in writing to Professor Candy McCabe, consultant nurse, Dr Peter Brook, consultant anaesthetist, or Dr Ashok Bhalla, consultant rheumatologist.
Patients aged 18 years or over with a possible diagnosis of CRPS will be considered via referral from medical personnel, and supported by the patient's GP. For those patients outside our regional funding agreements then funding approval will be required prior to any appointment.
We are also able to accept private patient referrals for those who are unable to access this service via the NHS funding routes. This will enable the purchaser to access the normal NHS service and will not expedite referrals, guarantee a single room or any other additional services outside of normal routine care. For further information please email email@example.com
Telephone advice is available for further information when considering a referral, ongoing advice for a patient already under our care or if another member of the primary care team (e.g., therapists) wishes to consult with the equivalent professional in our team. Please contact Professor Candy McCabe, consultant nurse and CRPS service lead, tel 01225 465941 ext 346 or email firstname.lastname@example.org; or email email@example.com
